Below is the list of said documents under Section 23.1 (a) and 24.1 (a):

 

1. Registration Certification;

i. Local Merchant:

☐ Single Proprietorship - Department of Trade and Industry (DTI) Certificate

☐ Corporation / Partnership - Securities and Exchange Commission (SEC) Registration Certificate

i. Certificate of Incorporation / Partnership

ii. Articles of Incorporation / Partnership

iii. Amended Articles of Incorporation / Partnership

iv. If a Certificate of Filling, it must be uploaded with the above mentioned documents.

☐ Cooperative - Cooperative Development Authority (CDA) Certificate of Registration

ii. Foreign Merchant

☐ Business Registration Certificate issued by the country of the merchant and documents may be substituted by the appropriate equivalent documents in English.

 

2. Mayor's / Business Permit or its Equivalent Document;

3. Tax Clearance Certificate for Bidding Purposes (Pursuant to Executive Order No. 398);

4. Philippine Contractors Accreditation Board (PCAB) license and registration (if applicable); and

5. Audited Financial Statements (AFS)

☐ Manual Filing - AFS must have a stamped “Received” by the Bureau of Internal Revenue (BIR) or its duly accredited and authorized institutions.

☐ Online Filing - Email confirmation receipt from BIR in lieu of the manual “Received” stamping in accordance with Revenue Memorandum Circular (RMC) No. 49-2020.

☐ AFS must have the following:

a. Auditor’s Report / Statement of Management’s Responsibility

b. Statement of Financial Position / Balance Sheet
